Who we are.
Slater Chiropractic (“the practice”, “we”, “us”) is a chiropractic practice operated by Dr. Savannah Slater and located in St. Albert, Alberta. We are regulated by the College of Chiropractors of Alberta (CCOA).
What we collect.
- Personal identification information (name, contact details, date of birth)
- Health history, current concerns, medications, and prior treatment
- Examination findings, treatment notes, and care plans
- Insurance and billing information where you choose to provide it
- Communications with the practice through forms, email, or the booking platform
How we use it.
Information is used solely to provide you with chiropractic care, manage appointments, communicate with you about your care, fulfil regulatory record-keeping obligations under the CCOA, and — where applicable — process insurance claims you authorize.
Where it is stored.
Clinical records and appointment data are held in Jane App, a Canadian healthcare practice management platform with appropriate security and privacy controls. Email communications are held in standard secure email infrastructure. We do not sell, rent, or share your information for marketing purposes.
Your rights.
Under Alberta’s Personal Information Protection Act (PIPA) and CCOA standards, you have the right to access your records, request corrections to inaccurate information, withdraw consent for ongoing care, and request that we explain how your information has been used. To exercise these rights, please contact the practice in writing.
Cookies & this website.
This website uses minimal, first-party functional cookies only — to remember non-essential preferences such as menu state. We do not run advertising or third-party tracking scripts. Embedded booking is provided by Jane App and is governed by Jane’s own privacy policy.
